Two traditions, two kinds of explanation
Thai massage comes from Thailand and shiatsu from Japan. Both come with traditional explanations (energy lines, meridians) that are cultural frameworks, not measured anatomy. This page covers what a session is, what small studies found, and safety. The history and concepts live elsewhere on this site: Thai massage: tradition, evidence and safety and shiatsu and related East Asian bodywork. Evidence labels: promising but limited, insufficient evidence, traditional use only.Thai massage (Nuad Thai): what a session is
UNESCO inscribed Nuad Thai, traditional Thai massage, on its Representative List of the Intangible Cultural Heritage of Humanity in 2019. Its description says the practice is regarded as part of traditional Thai healthcare and involves manipulation in which the practitioner helps rebalance the body, energy and structure, based on a belief that illness can come from obstruction of energy flow along "sen" lines. [1] The practitioner uses hands, elbows, knees and feet, sometimes with herbal hot compresses. [1] The record says Nuad Thai has roots in village self-care, where massage healers treated muscle aches from work in the fields. [1]Details such as clothing, mat or table, and session length are not described in the sources we read, so ask the practitioner before you book. You can also ask for the session to be stopped or changed: the Ontario (Canada) massage regulator's consent standard says registered massage therapists must tell patients that treatment will be stopped or modified at any time at their request. [2]
Thai massage: what research shows
The sen lines and the four-element framework are traditional ideas; the studies below did not test them. A systematic review of six studies on chronic pain found before-and-after pain reductions of 25% to 80%, with improvements in disability, muscle tension, flexibility and anxiety, and the pain benefit appeared to hold for up to 15 weeks. [3] The authors said more research was needed on long-term benefit compared with control conditions. [3] Before-and-after changes cannot separate the massage from expectation, natural recovery or attention.A hospital trial in Bangkok randomized 120 outpatients with chronic non-specific low-back pain to Thai massage or joint mobilization for up to eight sessions. Pain and disability fell in both groups and the authors called them equally effective in the short term; there was no untreated control group, and the abstract reports constipation in 28.3% of patients without explaining it. [4] Label: promising but limited.
Shiatsu: what a session is
Shiatsu was developed in Japan and is practiced around the world. [5] A 2011 review describes it as using finger pressure, manipulations and stretches along Traditional Chinese Medicine meridians, similar to acupressure. [6] Practitioners dispute that comparison: a 2018 paper reports technical differences from acupressure in the type of pressure, thumb positioning and use of body weight. [7] Meridians are traditional concepts, not biomedical mechanisms.Cancer Research UK says that in shiatsu you lie on soft mats on the floor with your clothes on, while most other massage uses a table and covers you with a gown or towel. [8]
Shiatsu: what research shows
A 2011 systematic review searched 1,714 publications and found 9 shiatsu studies (1 randomized trial) alongside 71 acupressure studies; it judged the shiatsu evidence to be of insufficient quantity and quality, while acupressure looked more promising for pain, nausea and vomiting, and sleep. [6] The authors of the 2018 paper argue that acupressure studies should not stand in for shiatsu evidence. [7]In a Tokyo trial of 59 people with chronic low-back pain, four weekly one-hour shiatsu sessions plus standard care were compared with standard care alone. The main disability score did not differ significantly between groups at 4 or 8 weeks, though some secondary scores favored shiatsu at 8 weeks. [5] The control group had no comparable attention, so expectation may explain part of the result.
A 6-month observational study followed 948 shiatsu clients of 85 practitioners in Austria, Spain and the UK. Symptom scores improved, but there was no comparison group. [9] Label: insufficient evidence for shiatsu as a treatment.
Tui na and choosing a practitioner
Tui na is Chinese therapeutic massage, also an East Asian tradition. [10] It has its own guide, Tui Na, which this page does not repeat.NCCIH says 45 states and the District of Columbia regulate massage therapy, and advises asking about a therapist's training, experience and credentials. [10] Rules differ by place, and the sources we read do not say how Thai massage or shiatsu are regulated or whether their practitioners need a massage license. See massage practitioner qualifications.
Frequently asked questions
What does "sen" mean in Thai massage?
In the traditional Thai explanation, sen are lines understood to crisscross the body along which energy flows. [1] They are a traditional concept, not a biomedical finding.
Is Thai massage good for chronic pain?
A systematic review of six studies found pain reductions after treatment, but more research was needed on long-term benefit compared with control conditions. [3]
Is shiatsu the same as acupressure?
A 2011 review calls them similar, since both use finger pressure along meridians. [6] A 2018 paper argues they differ in pressure, thumb positioning and use of body weight. [7]
What happens in a shiatsu session?
Cancer Research UK says you lie on soft mats on the floor with your clothes on. [8]
Does shiatsu work for low-back pain?
One small Tokyo trial found mixed results: the main outcome did not differ significantly, while some secondary outcomes improved. [5]

Safety
Tell the practitioner about osteoporosis, joint problems, pregnancy, cancer treatment, blood thinners or recent surgery or injury before you start, and see massage safety and contraindications. If back pain comes with pain, tingling, weakness or numbness in both legs, or changes in bladder or bowel control, the UK NHS says to call 999 or go to A&E; in the US, call 911. [11]
