MBSR in Detail: What the Eight-Week Course Involves and How to Find a Qualified Teacher
What the eight-week MBSR course involves week to week, what trials found, who should check with a clinician first, and how to ask about a teacher's training.

Mindfulness-based stress reduction (MBSR) is a group course that teaches mindful meditation and also includes discussion sessions. [1] This page explains what taking the course involves, what trials found, and how to check a teacher. It is information only, and it does not list or recommend any teacher or center. The wider overview is on MBSR, MBCT and how programs are structured.
Where the course came from
Jon Kabat-Zinn described an early version in 1982: a 10-week stress reduction and relaxation program for chronic pain patients, with results reported for 51 people. [2] Today it is usually taught over 8 weeks. The story of its spread into medicine is on the Kabat-Zinn page.
What the course involves
Details differ between programs, so check the outline of any course you consider. In one US trial the standard protocol was described as 8 weekly classes of 2.5 hours, a day-long retreat class in week five or six, and 45 minutes of home practice a day. [3] In another trial, MBSR (mindfulness meditation and yoga) was given in 8 weekly 2-hour groups. [4]
The practices taught in the first trial were:
- Breath awareness: focusing attention on the breath and other body sensations. [3]
- A body scan: directing attention to one body part at a time and noticing how it feels. [3]
- Mindful movement: stretches and movements meant to bring awareness to the body. [3]
Home practice matters in these courses, and not everyone keeps up. In the back pain trial, 53.7% of participants attended 6 or more of the 8 sessions. [4] See body scan meditation for that practice on its own.
What research shows
| Question | What the source found | Strength of evidence |
|---|---|---|
| Stress and anxiety in healthy adults | 29 studies (2,668 people): large effects on stress and moderate effects on anxiety, with high variation [5] | Promising but limited |
| Anxiety disorders compared with a medication | 276 adults, 8 weeks: MBSR was not worse than escitalopram on a clinician-rated scale [6] | Promising; one trial |
| Anxiety disorders compared with CBT | In a meta-analysis, MBSR showed significantly lower effects than CBT, while ACT and MBCT were comparable [7] | Mixed |
| Chronic low back pain | 342 adults: 60.5% on MBSR improved on a disability scale at 26 weeks, versus 44.1% with usual care; no significant difference from CBT [4] | Promising but limited |
| Pain in general | NCCIH reports short-term improvement in low back pain but not fibromyalgia pain [1] | Mixed |
Two cautions apply. Trials of a course cannot hide who is in the class, so expectations can shape results. Some of the trials above compared MBSR with a waitlist or usual care rather than another active treatment. [8][4] The anxiety trial also screened out several groups of people, as described below. [3]
How to check a teacher's training
There is no single regulator. Proper evidence not found for a US license for mindfulness teachers. NCCIH advises asking about the instructor's training and experience. [1] In England, NICE says group mindfulness should preferably be led by 2 practitioners, at least one with therapy-specific training and competence. [12]
Useful questions to ask:
- What teacher training did you complete, and who supervises your teaching?
- Have you completed the course yourself, and do you keep up a personal practice?
- How is teaching quality assessed? Some training centers use published teaching assessment criteria called MBI:TAC. [13]
- What happens if practice becomes distressing, and can I take part in a modified way?
- What is the schedule, home practice expected and refund policy?
Universities such as Brown offer MBSR and MBCT teacher training certificate pathways. [14] Training that exists is not the same as a license, so ask. More in how to find a meditation teacher or class.
Frequently asked questions
How long is the MBSR course?
Eight weekly classes, with a day-long retreat in one described version and daily home practice. [3]
Is MBSR just meditation?
No. It teaches mindful meditation and also includes discussion sessions, and one version also taught yoga and mindful movement. [1][4]
Can I do MBSR online?
Online mindfulness programs show moderate effects on stress and depression, larger with guidance. Proper evidence not found, in the sources read, for a direct online versus in-person MBSR comparison. [15]
Is MBSR safe if I have anxiety or depression?
It was tested in adults with anxiety disorders and was not worse than escitalopram, but the trial screened out several conditions, so ask your clinician. [6][3]
How do I know a teacher is qualified?
There is no US license. Ask about training, supervision and how distress is handled. [1]

Who should check with a clinician first
The anxiety trial excluded people with lifetime bipolar disorder, psychotic disorders or obsessive compulsive disorder, and also those with current PTSD, substance use disorders or significant active suicidal ideation. [3] That does not mean MBSR is unsafe for them, only that the trial cannot speak to it. If any of these apply, or if you have severe depression or a history of trauma, ask your clinician before joining. NCCIH says not to use meditation to replace conventional care. [1]
Side effects and stop signs
In community MBSR classes (2,155 people) and three trials, MBSR did not lead to higher harm rates than waitlist. [8] In the anxiety trial, 15.4% of the MBSR group had a study-related adverse event, against 78.6% on escitalopram. [6] Still, in a study of three MBCT variants (96 people), lasting bad effects occurred in 6 to 14%. [9]
Tell the teacher and consider pausing if practice brings panic, feeling unreal or detached, flashbacks, or distress that lingers after class. Lasting bad effects in the MBCT study were linked with signs of hyperarousal and dissociation. [9] The NHS says mindfulness is not right for everyone and can make some people feel worse. [10] See when to stop or pause meditating. In the US, call or text 988 in a mental health crisis, or call 911 in an emergency. [11]
