Safety first: do not replace diabetes treatment
Type 2 diabetes is managed with food and activity changes and, for many people, medicines such as metformin, and sometimes insulin. [1] Low blood sugar (hypoglycemia) usually affects people with diabetes who take insulin or some other diabetes medicines. [2] NCCIH says ashwagandha might interact with medicines for diabetes. [3] Any herb that lowers blood sugar could add to the effect of your medicines, so tell your diabetes team before using one. Never stop or reduce a diabetes medicine because of something you read here. See also Ayurvedic herbs with diabetes, thyroid and blood pressure medicines. What the research covers
This page reports studies of named products and herbs. Ayurvedic dietary advice for weight and blood sugar is covered separately in Ayurvedic diets for weight and blood sugar.Type 2 diabetes
| Intervention | What the source found | Label |
|---|---|---|
| Ayurvedic treatments overall (Cochrane review, 2011) | Six trials of proprietary herbal mixtures and one of whole-system treatment (354 people, 3 to 6 months). Some mixtures lowered HbA1c or fasting blood sugar compared with placebo or no extra treatment, others did not. The authors could not draw definite conclusions and found insufficient evidence to recommend them in routine care. [4] | Insufficient evidence |
| NCCIH summary | A trial of 89 people suggested a five-herb formulation may help, but other researchers said inadequate study designs prevent firm conclusions. [5] | Insufficient evidence |
| A six-herb polyherbal capsule versus metformin | In a trial of 93 newly diagnosed adults over 24 weeks, blood sugar and HbA1c reductions were similar to metformin. [6] | Promising but limited (one trial, active comparator) |
| Fenugreek (methi) | A 2023 meta-analysis of 10 trials (706 people) found reductions in fasting glucose, post-meal glucose and HbA1c; mild stomach side effects occurred in some studies. [7] See fenugreek. | Promising but limited |
| Gymnema sylvestre | A 2021 meta-analysis of 10 studies (419 people) found lower fasting glucose and HbA1c, but compared results with baseline instead of with a control group, and variation between studies was very high. [8] | Insufficient evidence |
| Triphala | A review of 12 randomized trials (749 people) found lower fasting glucose in people with diabetes but not in people without; larger well-designed trials are needed. [9] See triphala. | Promising but limited |
A before-and-after comparison, as in the gymnema analysis, cannot separate the herb from other changes such as diet, regression to the average or expectation. That is why we rate it lower than a placebo-controlled result.
Nepal, where many people use Ayurvedic centers for primary care, has also tested a clinical guideline for diabetes care by Ayurvedic practitioners. A 2026 feasibility trial of 121 adults showed the approach could be tested in 14 centers; the effects on HbA1c and fasting glucose were not statistically significant, and the study was designed only to show whether a larger trial is possible. [10]
Weight
Proper evidence not found for Ayurvedic stand-alone weight-loss programs. We searched PubMed for Ayurveda and obesity reviews and for Ayurvedic herbal weight-loss trials and found no review or trial we could read, apart from the triphala review above. In that review, five trials reported lower body weight, body mass index and waist size in people with obesity, with the authors calling for larger trials. [9] These are mostly short trials of one herb, not evidence for a "weight-loss plan".The NHS says the way to lose weight is to eat and drink fewer calories than the body uses and to do more physical activity, aiming to lose steadily; it also notes that losing 5 to 10 percent of body weight can improve health. [11] See healthy weight basics and weight loss supplements and fat burners. Ayurvedic "cleanses" and fasting programs are not weight-loss treatments; see panchakarma and cleansing.
Safety and quality
- Blood sugar can drop too low if a herb adds to a medicine. Know the signs: the NHS lists hunger, dizziness, sweating, shaking, confusion and blurred vision. [2] Check your blood sugar if you can and follow your care plan.
- Some Ayurvedic products contain lead, mercury or arsenic at toxic levels. [5] See heavy metals and product quality.
- Avoid very strict, low-calorie diets without talking to a health professional, especially if you take insulin or other diabetes medicine. [12]
- Ashwagandha is not recommended before surgery, in pregnancy or breastfeeding, according to NCCIH. [3]

Frequently asked questions
Can Ayurveda cure diabetes?
No source we read says so. A Cochrane review found some herbal mixtures lowered blood sugar but could not recommend them for routine care. [4]
Is fenugreek good for blood sugar?
A 2023 meta-analysis found lower fasting glucose and HbA1c in trials of fenugreek, with mild stomach side effects in some. Tell your diabetes team before using it. [7]
Can I take Ayurvedic herbs with metformin or insulin?
Ask your clinician or pharmacist first, because blood sugar could fall too low. [2]
Does triphala help weight loss?
A review of randomized trials reported lower weight and waist size in obese patients in five trials, but larger well-designed trials are needed. [9]
Are Ayurvedic weight-loss programs proven?
Proper evidence not found. The NHS approach is fewer calories than you use and more physical activity. [11]
