Vitamin B12 and Energy: What Is Claimed and What Trials Show in People Without Deficiency

Checks the claim that vitamin B12 gives energy, separating its real role and deficiency symptoms from what trials found in people who are not deficient.

The question

"Vitamin B12 for energy" is one of the most common supplement claims. Two different questions hide inside it. Does being short of B12 cause tiredness? Possibly, and a clinician can test for it. Does extra B12 give more energy to someone who is not short of it? The trials we could read do not show that. This page is information only. Ongoing tiredness has many possible causes, so please see a clinician rather than self-treating.

Evidence labels used: established nutritional role / deficiency treatment, no good evidence of benefit (in people without deficiency), insufficient evidence, marketing claim and Proper evidence not found.

What B12 really does

The NHS says vitamin B12 helps the body make red blood cells and keep the nervous system healthy, release energy from food and use folate. [1] That wording is the source of many energy claims. It describes a role in normal metabolism. It does not say that extra B12 produces extra energy once the body has enough. Treat "supports energy metabolism" on a label as a statement about a normal role, not a promise.

If you are actually deficient

The NHS lists extreme tiredness and a lack of energy among the symptoms of B12 or folate deficiency, alongside pins and needles. [2] It also notes that some of these problems can occur without anaemia, [2] that diagnosis is often based on symptoms plus a blood test, [2] and that some problems can be irreversible if treatment is delayed. [2] Do not diagnose yourself from tiredness. Tiredness is also common in many unrelated conditions.

What trials found in people without deficiency

QuestionWhat the source foundStrength of evidence
Fatigue in a systematic reviewA 2021 review of 16 randomised trials (6,276 people) studied B12 alone or in B-complex. Only one study reported effects on idiopathic fatigue, so no analysis was possible. [3]Insufficient evidence
MoodThe same review found no overall effect of vitamin supplementation on measures of depression. [3]No good evidence of benefit
ThinkingThe review found no evidence for an effect of B12 alone or B complex on any subdomain of cognitive function outcomes. [3]No good evidence of benefit
Fatigue with normal B12 blood levelsA placebo-controlled trial in 95 outpatients with irritable bowel syndrome or inflammatory bowel disease, who had deactivating fatigue and normal B12 levels, gave 1000 mcg a day or placebo for 8 weeks. [4] The authors concluded that surplus B12 had no beneficial clinical effect on fatigue in these patients. [4]No good evidence of benefit (this group only)
Injections for mild symptomsIn a crossover trial of 20 singers without known or suspected deficiency, voice measures after B12 injection were no different from placebo. [5]No good evidence of benefit (one small trial)

The 2021 review looked at patients without overt B12 deficiency, [3] so these results do not apply to people who are deficient. The reverse also holds: improvement in a deficient person does not show benefit for someone with normal levels.

Gaps

Proper evidence not found for B12 supplements improving energy or general fatigue in healthy adults with normal B12. We searched PubMed on 2026-10-23 for randomised trials and systematic reviews of vitamin B12 and fatigue or energy, and the reviews and trials we could read are those in the table. Absence of evidence is not proof that no one benefits, and it is not proof of safety either.

Claims that B12 improves mood, memory or "brain fog" fall under the same limit: the 2021 review found no effect on depression measures or cognition in people without overt deficiency. [3]

Frequently asked questions

Does vitamin B12 give you energy?

If you are deficient, the NHS says many symptoms improve with treatment. [2] In people without deficiency, trials we read found no benefit for fatigue, mood or thinking. [3] [4]

Why do people feel better after a B12 shot?

We cannot say for any one person. In a placebo-controlled trial, singers improved by similar amounts after B12 and after salt-water injections. [5] That is consistent with a placebo response, though one small trial cannot prove it.

What are signs of low B12?

The NHS lists extreme tiredness, a lack of energy, pins and needles and other symptoms, but these also occur in other conditions. [2] Only a clinician can say, usually with a blood test. [2]

Is B12 energy "support" on a label proof it works?

No. The NHS says B12 helps release energy from food, but that describes a normal role in the body, not a boost beyond it. [1]

Can I take B12 for energy while pregnant or on medicines?

Ask a clinician or pharmacist first. Do not change any prescribed treatment on your own.
